Basal Implantology

There are a few limitations of conventional implants. Some of them include resorbed and atrophic maxillary and mandibular ridges and absence of adequate bone support. In such situations basal implantology plays its role.

What is the rationale behind placement of Basal implants?

The teeth are placed in alveolar bone of the jaws. This bone is less dense and is also known as crestal bone. The alveolar bone has a tendency to get resorbed after the loss of teeth. The basal bone is the part of the jaw bone that remains after resorption of the alveolar bone.The basal bone forms the base of the alveolar bone which holds the teeth

The implants placed in the basal bone offer a long lasting solution to missing teeth.

These implants are less prone to infection and offer good retention.

 Basal implantology is also known as cortical, bi cortical and strategic implantology.

What are the indications of basal implants?

  1. Any patient with one or multiple missing teeth.
  2. In cases where bone augmentation is not possible.
  3. Patients who have bone atrophy or reduced bone height and width. This situation of the bone could arise due to a long edentulous span of the patient or due to presence of chronic periodontal diseases.

        Bone atrophy is also seen in patients who have been wearing dentures for a long time.

Contraindications of basal implants

  1. Certain medical conditions: This could be a general contraindication of implants. Whenever a patient has been suffering from any chronic cardiac condition with myocardial infarction or some immune suppressing disorder. 
  2. Anatomical inhibitions: In certain conditions, there is deficiency of muscles and ligaments and balanced occlusion is difficult to achieve.

What are the advantages of Basal implants over other implant systems?

  1. Less invasive: Since this is a single piece implant system, it is done without raising a flap. When the surgical invasion is less, there are fewer chances of post operative swelling and complications.
  2. One step procedure: Unlike conventional implants which have a loading period of around 3-4months, basal implants are loaded with prosthesis in less than a week.
  3. Less chances of failure: There is no implant and abutment interface interaction as seen in conventional implants and hence it has a better success rate.
  4. Stronger bone support: These implants take support from the basal bone. The basal bone is stronger as compared to the crestal bone.
  5. Better distribution of the masticatory forces: The basal bone resorbs less and repairs faster than the crestal bone and hence it can take the load of biting force in a much better way.
  6. Fewer chances of peri implantitis: Inflammation around the implants is known as peri implantitis. This is the most common cause of failure of implants. The basal implants are monoblock and smooth and for this reason there is little or no inflammation around the implants after they have been placed.
  7. Shows better results in medically compromised patients:  Basal implant system has shown better success rate in patients who have chronic diabetes and periodontal problems.
